HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
- Location: Fayetteville WV 25840 – 100% On-site

- Counties Covered: Nicholas and Fayette
- Payrate: $57.30/hr + mileage
- Required Availability: Full-Time | Monday–Friday, 8:00 AM–5:00 PM

- On-Call: One Saturday per month
HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Physical Therapist role provides in-home rehabilitation services to help patients improve mobility, function, and overall independence. The therapist evaluates patient needs, develops individualized treatment plans, and collaborates with the care team to deliver high-quality, compliant care across the assigned service area.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
- Perform comprehensive physical therapy evaluations and treatments based on physician referrals
- Review patient medical histories, diagnoses, and precautions to develop individualized care plans
- Establish measurable goals and treatment plans to support functional independence
- Reassess patient progress regularly and adjust treatment plans as needed
- Communicate patient progress and needs with physicians, care teams, patients, and families
- Recommend and assist with adaptive equipment to enhance rehabilitation outcomes
- Maintain accurate and timely clinical, treatment, and billing documentation in compliance with all regulations
- Supervise Physical Therapist Assistants and Physical Therapy Aides in accordance with state practice guidelines
- Participate in staff development, meetings, and continuing education to maintain clinical excellence
HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active West Virginia Physical Therapist license
- Compact licenses accepted
- Home health experience required
