- Plans, implements, and evaluates patient care plans to restore or maintain patient well being.
- Performs clinical assessments to identify patient needs and appropriateness.
- Documents and reviews a Plan of Care (POC) and progress notes on an ongoing basis.
- Provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
- Instructs patients and families in transfer techniques and use of therapy equipment.
- Educates patients/families regarding therapeutic procedures based on individual needs.
- Applies concepts of infection control in coordinating and performing patient care activities.
- Requires local travel to serviced areas.
- Flexibility in schedule is necessary.
- Current State License as a Physical Therapist - Required.
- Proof of current CPR certification - Required.
- Bachelor of Science in Physical Therapy from an accredited program - Required.
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required.
- Benefits are available to full-time employees after 90 days of employment.
