- Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
- Documents and prepares clinical records, revises Plans of Care based on ongoing assessment.
- Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders.
- Instructs patients and families in transfer techniques and safe use of equipment.

- Strong background in homecare and experience with Homecare Homebase preferred.
- Bachelor of Science in Physical Therapy from an APTA approved program - Required.
- Current State License as a Physical Therapist - Required.
- Proof of current CPR certification - Required.
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required.
