HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
- Location: Ft Wayne, IN 46805 – 100% On-site, home health coverage

- Coverage Area: Noble, Lagrange, Steuben, Dekalb, Allen
- Payrate: $64.04/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- Weekends available, but not required
- Target: 30 points per week, Average caseload: 5–6 patients per day
HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Physical Therapist will provide in-home physical therapy evaluations and treatments based on physician referrals, developing and adjusting individualized care plans to help patients regain function and independence. Collaborates with physicians, care teams, patients, and families while ensuring accurate documentation, regulatory compliance, and supervision of therapy support staff.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
- Perform comprehensive physical therapy evaluations and assessments
- Develop and implement individualized treatment plans based on physician referrals
- Establish measurable goals and regularly reassess patient progress
- Modify treatment plans as clinically indicated
- Communicate patient progress with physicians, care teams, patients, and families
- Recommend and assist with adaptive equipment to improve patient function
- Complete accurate and timely clinical and billing documentation
- Ensure compliance with professional practice standards and applicable regulations
- Supervise Physical Therapist Assistants and Therapy Aides as required
- Participate in staff meetings, training, and continuing education
HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable) required
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Indiana Physical Therapist license (compact license accepted)
- Home health experience required
- Proficiency with OASIS/HCHB documentation
- Strong skills in exercise programming and rehabilitation
- Ability to work independently and manage a flexible schedule
