HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
- Location: Greenville AL 36037 – 100% On-site
- Payrate: $57.30/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Physical Therapist role focuses on evaluating, treating, and supporting patients in achieving their highest level of functional independence through personalized rehabilitation plans.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
- Perform physical therapy evaluations and treatments based on physician orders
- Complete comprehensive assessments using appropriate clinical tools
- Review patient medical histories, diagnoses, and precautions
- Develop and implement individualized treatment plans with measurable goals
- Reassess patient progress regularly and modify care plans as needed
- Communicate patient progress with physicians, care teams, and family members
- Recommend and assist with adaptive equipment to improve patient function
- Maintain accurate and timely clinical and billing documentation per regulations
- Supervise Physical Therapist Assistants and Therapy Aides as applicable
- Participate in staff development, meetings, and continuing education activities
- Adhere to all professional, ethical, and regulatory standards
HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Physical Therapist license in Alabama or compact required
- Preferred: Home health experience, OASIS documentation, Wound care, HCHB (Homecare Homebase), Exercise planning and patient rehabilitation
