HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
  • Location: Griffin GA 30224 – 100% On-site
    • On-site, home health visits across Spalding, Pike, and Henry counties
  • Payrate: $60.67/hr + $0.49/mile
  • Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:

The Home Health Physical Therapist will provide skilled evaluation and treatment for patients in a home health setting. The therapist will develop and implement individualized treatment plans to maximize patient independence and functional outcomes.
  • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
  • Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
  • Conduct comprehensive evaluations using assessment tools and procedures
  • Review patient records for current diagnosis, medical history, precautions, and contraindications
  • Establish measurable goals and develop treatment plans to promote maximum independence
  • Reassess treatment effectiveness and modify goals or interventions as needed
  • Communicate patient progress and needs to the care team, physicians, and family members
  • Consult with medical providers regarding treatment, progress, and discharge planning
  • Recommend and assist in obtaining adaptive equipment to enhance patient function
  • Supervise Physical Therapist Assistants and Physical Therapy Aides in patient care
  • Maintain accurate documentation for evaluations, treatments, and billing per regulations
  • Participate in staff development programs and maintain continuing education requirements

HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
  • Education: High School Diploma or GED (verifiable)
  • Attendance: Commitment to mandatory attendance during the first 90 days.
  • Active Physical Therapist license
  • Experience in home health care
  • Strong documentation and communication skills
  • Ability to travel to patient homes within assigned counties
  • Preferred: Familiarity with Homecare Homebase (HCHB), Prior experience supervising PTAs and aides
