HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
- Location: Knoxville TN 37922 – 100% On-site
- Payrate: $55.95/hr + mileage reimbursement
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- No required weekends (optional if you want them)
- No on-call or overtime requirements
HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Physical Therapist role focuses on providing high-quality, patient-centered care in the home setting while supporting functional independence and recovery.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
- Perform physical therapy evaluations and treatments based on physician referrals
- Complete comprehensive patient assessments using appropriate clinical tools
- Review medical histories, diagnoses, and precautions prior to treatment
- Develop individualized treatment plans with measurable goals
- Reassess patient progress regularly and adjust plans as needed
- Communicate patient progress with physicians, care teams, and family members
- Recommend and assist with adaptive equipment to improve patient function
- Maintain timely and accurate clinical and billing documentation
- Supervise Physical Therapist Assistants and Therapy Aides in accordance with state regulations
- Participate in staff meetings, education, and ongoing professional development
HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Tennessee Physical Therapist license or compact license
- Home Health experience required
- Homecare Homebase (HCHB) experience required
- Vestibular therapy experience (nice to have, not required)
