HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
- Location: Lakewood CO 80228 – 100% on-site

- Field-based coverage across Denver, Adams, Jefferson, Douglas, Boulder, Arapahoe, Broomfield, and Larimer counties.
- Payrate: $57.30/hr + $0.45/mile
- Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM

- Productivity: 30 points per week.
HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Physical Therapist performs comprehensive clinical evaluations and develops individualized treatment plans to help patients achieve maximum functional independence across the Front Range region. This role is responsible for supervising assistant staff, managing a field-based caseload across several counties, and ensuring all OASIS and billing documentation meets strict regulatory and company standards.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
- Evaluations & Treatment: Conduct comprehensive physical therapy evaluations using standardized assessment tools; review patient medical records to identify precautions and contraindications.
- Care Planning: Establish measurable goals and develop individualized treatment plans designed to maximize patient independence and functional mobility.
- Clinical Re-assessment: Regularly monitor the effectiveness of treatment plans and patient progress, adjusting goals or interventions as necessary based on clinical outcomes.
- Interdisciplinary Collaboration: Communicate patient needs and progress clearly to the treatment team, physicians, and family members.
- Professional Supervision: Oversee and guide Physical Therapist Assistants (PTAs) and Physical Therapy Aides in direct patient care activities.
- Compliance: Maintain and submit accurate evaluation, treatment, and billing documentation within required timelines and in accordance with state and federal regulations.
HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
- Education: Doctor of Physical Therapy (DPT) degree is required
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Licensure: Current, unrestricted Physical Therapist license for the state of Colorado.
- Transportation: Must have a reliable, working vehicle for travel within the specified service area.
- Strict adherence to HIPAA, Corporate Compliance programs, and the Company’s Code of Business Conduct.
Preferred Skills
- Technical Proficiency: Prior experience with OASIS documentation and Home Care Home Base (HCHB) software is highly preferred.
- Clinical Experience: Proven ability to manage a field-based caseload while meeting productivity standards.
