Job Description
- Performs physical therapy evaluation and treatment of patients based upon physician orders/referrals.
- Completes a comprehensive evaluation using appropriate tools and procedures.
- Establishes measurable goals and develops treatment plans to improve patient independence and function.
- Regularly reassesses treatment plan effectiveness and modifies goals as necessary.
- Communicates patient needs and progress to the treatment team and family members.
- Maintains and submits evaluation, treatment, and billing documentation in compliance with regulations.
- Supervises Physical Therapist Assistants and Aides in direct patient care.
- Shift Requirements: Monday - Friday 8am - 5pm.
- On-site work location.
- PA License required.
- Experience with HCHB and Oasis preferred.
- Benefits are available to full-time employees after 90 days of employment.
