PHYSICAL THERAPIST (HOME HEALTH) | DETAILS AND COMPENSATION:
- Location: Owensboro, KY – 100% On-site, home health setting

- Coverage area : Daviess, Ohio, McLean, Henderson, Webster counties
- Payrate: $64.04/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
PHYSICAL THERAPIST (HOME HEALTH) | SUMMARY AND HIGHLIGHTS:
The Physical Therapist (Home Health) will provide high-quality, patient-centered care in an onsite setting. This role focuses on evaluating patients, developing individualized treatment plans, and supporting patients in achieving their highest level of independence and functional mobility.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

PHYSICAL THERAPIST (HOME HEALTH) | RESPONSIBILITIES:
- Perform physical therapy evaluations and treatments based on physician referrals
- Complete comprehensive assessments using appropriate tools and techniques
- Review patient medical history, diagnoses, precautions, and contraindications
- Develop measurable goals and individualized treatment plans
- Reassess patient progress regularly and adjust treatment plans as needed
- Communicate patient progress and care needs with the interdisciplinary team, physicians, and family members as appropriate
- Recommend and assist with adaptive equipment to improve patient function
- Maintain timely and accurate evaluation, treatment, and billing documentation in compliance with company, state, and federal regulations
- Supervise Physical Therapist Assistants and Physical Therapy Aides in accordance with state practice acts
- Participate in staff development, meetings, and continuing education to maintain and enhance clinical skills
PHYSICAL THERAPIST (HOME HEALTH) | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Kentucky Physical Therapist license or eligibility through compact licensure
- Ability to work onsite
- Strong clinical, communication, and documentation skills
Preferred Skills
- Home Health or Home-Based Care experience
- Familiarity with Homecare Homebase (HCHB)
- Medicare OASIS experience
