HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
- Location: Pueblo CO 81008 – 100% on-site, home health settings

- Field-based coverage for Pueblo, Fremont, Crowley, Custer, El Paso, Otero, Bent, Huerfano, and Las Animas counties.
- Payrate: $50.56/hr + $0.45/mile
- Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM

- Productivity: 30 points per week.
HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Physical Therapist performs comprehensive clinical evaluations and develops individualized treatment plans to help patients achieve their maximum level of functional independence across a multi-county service area. This role is responsible for supervising assistant staff, recommending adaptive equipment, and managing all aspects of documentation and billing in accordance with state and federal regulations.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
- Comprehensive Evaluations: Perform detailed physical therapy evaluations and treatments based on physician referrals, utilizing appropriate assessment tools and reviewing medical history for precautions.
- Care Planning: Establish measurable goals and develop individualized treatment plans to improve patient independence and function.
- Continuous Assessment: Regularly re-evaluate the effectiveness of treatment plans and patient progress, modifying goals or interventions as necessary.
- Team Collaboration: Communicate patient needs and progress effectively to physicians, treatment teams, and family members.
- Clinical Supervision: Supervise Physical Therapist Assistants (PTAs) and Aides in direct patient care activities according to the state practice act.
- Equipment Coordination: Recommend and assist in securing adaptive equipment to enhance the rehabilitation program and improve patient function.
- Regulatory Compliance: Maintain and submit accurate evaluation, treatment, and billing documentation in accordance with local, state, and federal regulations.
HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
- Education: Must hold a PT Doctorate License
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Professionalism: Adherence to HIPAA, corporate compliance programs, and professional practice standards.
Preferred Skills
- Point of Care Proficiency: Fluency with point-of-care documentation systems is highly preferred.
- Self-Management: Strong organizational skills to manage a field-based schedule and meet productivity requirements.
