HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:
- Location: South Ronceverte WV 24970 – 100% On-site

- Pocahontas County, West Virginia
- Payrate: $57.30/hr + mileage
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- No weekends, no on-call
HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Physical Therapist will provide high-quality patient care in a home health setting. This role focuses on evaluating, treating, and supporting patients to help them achieve maximum functional independence.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
- Perform physical therapy evaluations and treatments based on physician referrals
- Complete comprehensive assessments using appropriate tools and clinical judgment
- Review patient medical history, diagnoses, precautions, and contraindications
- Develop individualized treatment plans with measurable goals
- Reassess patient progress regularly and adjust plans as needed
- Communicate patient progress with physicians, care teams, patients, and families
- Recommend and assist with adaptive equipment to improve patient function
- Maintain timely and accurate clinical and billing documentation
- Adhere to professional standards, compliance guidelines, and applicable regulations
- Supervise Physical Therapist Assistants and Aides in accordance with state practice acts
- Participate in staff development and complete required continuing education
HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active West Virginia Physical Therapist license
- Ability to work independently in a home health environment
- Preferred: Home Health Physical Therapy, OASIS documentation, Homecare software experience (HCHB), Exercise program development, Rehab experience
