Job Title: Physical Therapist
Location: Portland, ME 04106
Shift: MON-FRI until shift is complete. Guaranteed Hours: 32 hours.
Pay: $55.00/hr.
Scheduled Pay: Bi-Weekly

Physical Therapist Description/Duties
A registered professional who provides physical therapy to patients of all age groups. Plans implements and evaluates patient care plans to restore or maintain patient well-being. Plans organizes and conducts physical therapy treatment program based on the medical referral and their evaluation. Collaborates with interdisciplinary team.
- Performs clinical assessment to identify patient needs and appropriateness of patient
- Document prepare/review and complete a Plan of Care POC progress notes phone orders and other clinical record documentation on an ongoing basis.
- Revise the POC based on evaluation ongoing assessment data.
- Works in consultation with the physician and as part of an interdisciplinary team.
Job Requirements
Physical Therapist Requirements
- Current State License as a Physical Therapist-Required
- Proof of current CPR certification-Required
- Current driver's license. Must have reliable transportation and insurance-Required
- 1-2 years’ experience as a Physical Therapist in an acute care or rehabilitation setting-Required
- Previous, recent home care experience-Required
- Education Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program-Required
- 1 year experience with Homecare Homebase Home Health Care
- 1 years’ experience with OASIS (Outcome and Assessment Information Set)
