Home Health Physical Therapist (PT)
Pay Rate: $64.04 per hour
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (No On-Call)
Mileage Reimbursement: $0.43 per mile
Territory Covered: Lowndes, Echols, Brooks, Cook, Berrien, and Lanier Counties (GA)
License Required: Active Georgia PT License or Compact License
Experience Preferred: 2+ years Home Health; HomeCare Homebase experience strongly preferred

Key Responsibilities

Perform comprehensive physical therapy evaluations based on physician referrals

Review medical history, diagnoses, precautions, and contraindications

Develop measurable treatment goals and individualized care plans

Reassess patient progress regularly and modify treatment plans as needed

Communicate effectively with physicians, interdisciplinary teams, patients, and families
-
Recommend and assist patients with adaptive equipment to enhance function
-
Maintain accurate, timely clinical and billing documentation in compliance with company, state, and federal guidelines

Supervise Physical Therapist Assistants (PTAs) and Physical Therapy Aides in accordance with state practice act

Participate in staff meetings, training, and continuing education programs
Qualifications

Doctor of Physical Therapy (DPT) or equivalent
-
Active Georgia Physical Therapist license (or Compact license accepted)

Minimum 2 years of home health experience preferred

Experience with HomeCare Homebase strongly preferred

Strong clinical assessment and communication skills

Ability to work independently and manage a patient caseload effectively
