Primary Care Provider / Family Nurse Practitioner (FNP)
Looking for an FNP to work as a Primary Care Provider in a clinic setting.
Schedule:
- Clinic hours, minimum 30/hrs per week. Requires working every other weekend and some major holidays
- 10-hour shifts M-F, 8 hours on the Weekends
Primary Care Family Nurse Practitioner (FNP) Requirements:
- Completion of a master’s degree level Family Nurse Practitioner program with current National Board Certification
- State of Employment license to practice in the Advanced Practice Nurse
- A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner/Physician Assistant in either a family medicine or primary care practice preferred.
Primary Care Family Nurse Practitioner (FNP) Job Duties:
- Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients
- Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.
- Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
- Counsels and educates patients and families on health maintenance and respond to patient care inquiries
- Evaluates and treats patients' medical needs with safe and high-quality treatment
- Documents all patient care within an electronic health record in accordance with current policies and procedures
- Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients
- Provides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease
- Refers to appropriate specialists and patient care components as appropriate
- Collaborates with clinical care team to ensure the delivery of high quality, patient centric, longitudinal care
- Resolves conflict using appropriate communication and management techniques
- Cultivates and maintains positive relationships with clinical care team members
