Job Title: Program Specialist - Aging and Disability Resources Program

Location: Home-based within the assigned northwest region of Pennsylvania (Remote with Travel)
Work Schedule: Full-time, Monday to Friday, 37.5 hours/week (flexible schedule available)
Travel: Approximately 70% travel within the designated area; occasional travel beyond the region may be required.

Position Overview:

The Program Specialist will support the Aging and Disability Resources program by providing ongoing technical assistance and coordination for three Service Area networks in the northwest region. This role is essential for fostering partnerships and ensuring that individuals over 60 and those with disabilities can access long-term services and supports (LTSS).

Service Areas:

  • Service Area 1: Clarion, Crawford, Erie, Forest, Venango, Warren
  • Service Area 4: Cameron, Clearfield, Elk, Jefferson, McKean, Potter
  • Service Area 7: Bradford, Sullivan, Susquehanna, Tioga

Key Responsibilities:

  • Organizational Skills: Work independently while managing multiple tasks and responsibilities.
  • Relationship Building: Develop and maintain relationships with stakeholders to enhance the success in the region.
  • Program Monitoring: Oversee program objectives by evaluating policies, guidelines, and budgets.
    • Provide guidance to Service Area Oversight Committees and Lead Coordinators.
    • Facilitate network collaboration and partner development.
    • Monitor developmental plans for Lead Coordinators.
  • Person-Centered Counseling (PCC): Support PCC activities by organizing training and addressing ongoing educational needs.
    • Ensure compliance with reimbursement standards.
  • Operations Management: Communicate program progress to the Program Manager and resolve issues within service areas.
  • Meeting Coordination: Organize internal and external meetings, including presentations and status updates.
  • Policy Development: Assist with developing and implementing policies and procedures.
  • Compliance Analysis: Analyze service area budgets and activity reports for adherence to standards.

Qualifications:

  • One (1) year of professional experience in developing, implementing or evaluating human services, sociology, public health, health care services, or health care insurance programs; and a bachelor's degree in Human Services or Education Field; or
  • Any equivalent combination of experience and training that includes at least one (1) year of professional experience in developing, implementing or evaluating human services, education, sociology, public health, health care services, or health care insurance programs.
  • Strong organizational skills and ability to work independently.
  • Experience in program coordination, relationship management, or a related field.
  • Knowledge of aging and disability services is a plus.
