RN CASE MANAGER - HOME HEALTH | DETAILS AND COMPENSATION:
- Location: Batesville, AR – 100% On-site, field work
- Coverage are : Independence, Izard, Sharp, Jackson, White, Cleburne, Lawerence, Randolph, Fulton, Woodruff, Craighead Counties
- Payrate: $56.14/hr + mileage
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
- On Call: Rotational weekend call (once per month)
RN CASE MANAGER - HOME HEALTH | RESPONSIBILITIES:
- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Identify and continuously reassess appropriate and cost-effective levels of care
- Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals
- Arrange non-medical support services (e.g., housing, transportation) to support treatment compliance
- Engage specialty resources as needed to resolve complex care needs
- Maintain detailed documentation of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Prevent adverse patient events when possible and intervene promptly when necessary
- Act as a patient advocate while ensuring privacy and confidentiality
RN CASE MANAGER - HOME HEALTH | REQUIREMENTS:
- Education: Associate’s or Bachelor’s Degree in Nursing (or related field)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active, unrestricted Registered Nurse license (Arkansas or compact license accepted)
- Case Management certification preferred
- Strong clinical assessment and care coordination skills
- Ability to analyze complex medical information and make sound clinical decisions
- Experience in case management or community-based care preferred
- Home health experience is a plus

- HCHB (Homecare Homebase)
- OASIS documentation
- Catheter care
- IV infusion therapy
- Wound care management
