RN Case Manager | Details and Compensation:
- Location: Manchester, TN 37355 – Field work required
- Payrate: $56.14/hr
- Required Availability: Mon-Fri 8a-5pm
RN Case Manager | Summary and Highlights:
The Hospice Registered Nurse (RN) Case Manager will be responsible for managing a caseload of hospice patients, providing compassionate care, coordinating services, and ensuring individualized care plans are implemented effectively. This position involves daily patient visits and requires participation in an on-call rotation.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

RN Case Manager | Responsibilities:
- Conduct comprehensive assessments of hospice patients.
- Collaborate effectively with primary care providers, physicians, social workers, and other healthcare professionals to develop and implement individualized, comprehensive care plans.
- Focus on cost-efficient and appropriate levels of care, reassessing patient needs continuously.
- Coordinate necessary non-medical support for patients, such as housing and transportation, to ensure compliance with treatment plans.
- Manage care transitions smoothly, coordinating with discharge planners and other relevant parties.
- Engage specialty resources as needed to achieve optimal patient outcomes.
- Maintain detailed and accurate records of clinical, functional, and fiscal outcomes.
- Identify opportunities for health promotion and illness prevention within the hospice setting.
- Act as a strong patient advocate, protecting privacy and confidentiality.
- Typically conduct around 5 patient visits per day.
- Participation in an on-call rotation is required. This may include covering evenings, overnight shifts, 24-hour periods, or entire weekends as needed.

RN Case Manager | Requirements:
- Education: Associate's or Bachelor's Degree in Nursing
- Current, unrestricted Registered Nurse (RN) license in the state of TN.
- Ability to analyze complex medical information and make rational decisions.
- Case Management certification is required.
- Experience with Home Care Home Base (HCHB) electronic medical record system is HIGHLY PREFERRED.
- Candidates without HCHB experience must be highly tech-savvy and able to learn new software systems quickly.
- Attendance: Commitment to mandatory attendance during the first 90 days.
