Job Description

Job Title: RN Case Manager

Pay: $48-$52 an hr | Bi-weekly

Job Type:

  • Onsite in Aurora CO 80045

Shift Options:

  • The work will be performed Monday-Friday, excluding federal holidays; 8-hour shift between the hours of 7am-530pm Mountain Time

RN Case Manager Overview:

  • Care Coordination: Assess, plan, implement, and evaluate comprehensive care plans for Veterans, ensuring seamless coordination between VA facilities and community providers. USAJOBS

  • Consult Management: Process community care consults, following them from initiation to completion, and address follow-up requests for continued or additional care needs. USAJOBS

  • Interdisciplinary Collaboration: Work closely with internal and external healthcare professionals to facilitate high-quality, cost-effective patient care. USAJOBS

  • Patient Advocacy: Serve as a liaison and advocate for Veterans and their families, ensuring their healthcare needs are met with dignity and compassion. USAJOBS

  • Utilization Management: Monitor and evaluate the effectiveness of care plans, ensuring appropriate resource utilization and adherence to clinical guidelines.

  • Documentation: Maintain accurate and timely documentation in accordance with VA policies and procedures.

RN Case Manager Qualifications:

  • A minimum of two (2) years direct patient care experience within the last three (3) years
  • Associate's or Bachelor's degree in Nursing from an accredited educational institution.
  • All Registered Nurse applicants must possess and maintain an active, current, full and unrestricted Registered Nurse license to practice as a RN in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
  • Active BLS (AHA) certification.
  • Proficiency in reading, writing, speaking, and understanding English.
  • Basic computer skills, including typing.

RN Case Manager Benefits:

  • Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance.
