Registered Nurse Case Manager Compensation

• The pay for this position is $44 - $47 per Hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Registered Nurse Case Manager Highlights

• The required availability for this position is Monday – Friday from 8AM – 5PM

Registered Nurse Case Manager Responsibilities

• Care Manager would be responsible to complete assessments per week with our members in their home.

• Perform self- scheduled assessments timely base from reports provided by leadership, complete visit documentation timely.

• Evaluate members for possible community relocations.

• Review task queue and complete tasks as needed. Make follow up attempts as required, collaborate with nursing facility staff to ensure that members are safe, obtain required items to meet member needs, and obtain member satisfaction to meet compliance and contractual requirements.

• Complete in-person follow up with members and provide updates for in-jeopardy investigations.

• Attend scheduled team meetings.

• Complete assigned mandatory HHSC Centene University trainings.

Registered Nurse Case Manager Requirements

• TX RN License
• Case Management experience
• Experience with electronic medical records
• Attendance is mandatory for the first 90 days

Registered Nurse Case Manager Preferences

• Bachelor’s Degree
• Home Health experience
• Long Term Care experience
