RN CASE MANAGER | DETAILS AND COMPENSATION:
- Location: Lexington, KY – 100% On-site, field work
- Payrate: $56.16/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
RN CASE MANAGER | SUMMARY AND HIGHLIGHTS:
The RN Case Manager will support high-risk members through comprehensive assessments, care coordination, and individualized care planning. This hybrid, field-based role focuses on improving patient outcomes through collaboration, advocacy, and efficient care management.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN CASE MANAGER | RESPONSIBILITIES:
- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Identify and reassess appropriate, cost-effective levels of care
- Coordinate care transitions across the continuum of care
- Arrange non-medical support services such as housing and transportation
- Partner with physicians, social workers, discharge planners, and other care team members
- Engage specialty resources to support optimal patient outcomes
- Maintain detailed documentation of clinical, functional, and financial outcomes
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate while ensuring privacy and confidentiality
RN CASE MANAGER | REQUIREMENTS:
- Education: Associate’s or Bachelor’s Degree in Nursing or related field
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active, unrestricted Registered Nurse (RN) license in the state of practice
- Strong clinical judgment and ability to analyze complex medical information
- Case Management certification preferred
