RN Case Manager
Job Description
Title: RN Case Manager – Home Health (OASIS + HCHB) – Lexington, KY
Location: Lexington, KY (On-site/field-based)
Schedule: Monday–Friday, 8:00 AM–5:00 PM; on-call 1 weekend/month + 1 day/week
Pay: $56.14/hour + mileage reimbursement
Summary
We are hiring an experienced RN Case Manager in Lexington, KY to provide home health case management and care coordination for high-risk members. This RN role focuses on comprehensive assessments, individualized care plans, cost-effective levels of care, and safe transitions across the continuum. The ideal RN has strong Home Health, OASIS, and HCHB (Homecare Homebase) experience and can document outcomes while advocating for patient safety, privacy, and confidentiality.
Responsibilities
- Assess high-risk members and document clinical/functional needs
- Collaborate with PCPs and interdisciplinary partners to implement care plans
- Coordinate care transitions and engage specialty resources as needed
- Identify health promotion and prevention opportunities; reduce avoidable events
- Maintain detailed documentation of outcomes and progress
- Support compliance with care plans by arranging non-medical resources
- Manage field-based coverage across multiple counties (mileage reimbursed)
Qualifications
- Active RN license (KY or compact)
- Case Management certification (CCM/ACM/ACM-RN/RN-BC/CMGT-BC)
- Home health case management experience
- OASIS documentation experience (SOC/recert/ROC)
- HCHB (Homecare Homebase) experience
- Able to take on-call rotation
