Job Posting: Registered Nurse (Field RN Case Manager)

Location: Pleasant View, TN 37146

Hourly Pay: $48.69/hr.

Shift: 8:00 AM – 4:30 PM, Monday to Friday
Weekend On-Call: One weekend on-call per month required

Key Responsibilities:

• Patient Assessment & Care Planning: Observe, assess, plan, implement, and evaluate nursing care for well, chronic, or acutely ill individuals in home and small facility settings.
• Care Coordination: Work closely with other healthcare professionals to plan, implement, and evaluate patient care programs.
• Medication Management: Order, review, monitor, discontinue, and, when necessary, dispose of medications as prescribed by the physician. Ensure adherence to medication policies and procedures, including annual reviews.
• Progress Reporting: Communicate changes in patient conditions to physicians and ensure physician orders are transcribed accurately.
• Documentation: Complete daily, monthly, quarterly, and annual nursing documents as required. Maintain up-to-date records of patients' health status in nurse’s notes.
• Staff Supervision: Supervise the activities of Licensed Practical/Vocational Nurses (LPN/LVN) as needed. Provide training and ongoing education on health and safety-related topics.
• Health & Safety: Ensure that health-related appointments are scheduled and completed. Address any findings in a timely manner to maintain patients' health and safety.
• Training & Education: Provide new employee training and annual recertification on health and safety-related topics.
• Collaboration: Maintain communication with inpatient nursing staff and discharge coordinators regarding patient hospitalizations.

Requirements:

• Licensure: Active Tennessee RN License
• Experience: Must have Home Health experience and be proficient in using Oasis Assessment Form.
• Technologically Savvy: Comfortable using technology; experience with HCHB (Homecare Homebase) is a plus, though not required.
• Travel: Ability to travel between patient locations; mileage is reimbursed at $0.41 per mile.
• Weekend Availability: One weekend on-call per month is required.

Desired Qualifications:

• Experience with HCHB (Homecare Homebase) is highly recommended.
• Strong case management experience, particularly in the home health environment.
• Ability to supervise and manage care plans and staff efficiently.
