Registered Nurse (RN) – Case Manager Compensation
- Competitive pay rate: $56.14 per hour
- Mileage reimbursement: $0.46 per mile
- Coverage are : Independence, Izard, Sharp, Jackson, White, Cleburne, Lawerence, Randolph, Fulton, Woodruff, Craighead Counties
Registered Nurse (RN) – Case Manager Highlights

Onsite role

Full-time, 40 hours per week

Monday–Friday, 8:00 AM–5:00 PM

Weekend on-call rotation required

RN – Case Manager Responsibilities
-
Perform comprehensive assessments of high-risk members
-
Collaborate with primary care providers to implement individualized, comprehensive care plans
-
Identify, address, and continuously reassess appropriate and cost-effective levels of care
-
Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
-
Engage specialty and community resources to support treatment compliance (housing, transportation, etc.)
-
Maintain detailed records of clinical, functional, and fiscal outcomes
-
Identify opportunities for health promotion and illness prevention
-
Prevent adverse patient outcomes and intervene quickly when necessary
-
Act as a patient advocate while maintaining privacy and confidentiality standards
Registered Nurse (RN) – Case Manager Requirements
-
Associate’s or Bachelor’s Degree in Nursing or related field
-
Current, full, active, and unrestricted RN license in Indiana (compact license accepted)
-
Case Management certification
-
Ability to analyze complex medical information and make sound clinical decisions
Preferred Qualifications
-
Wound care experience
-
IV therapy experience
-
Drain and G-tube care
-
Patient education experience
-
Experience with Homecare Homebase (HCHB) preferred
