HOME HEALTH RN CASE MANAGER | DETAILS AND COMPENSATION:
- Location: Jeffersonville IN 47130 – 100% on-site, home health settings
- (Coverage for Harrison, Crawford, Orange, Floyd, and Washington Counties)
- Payrate: $56.14/hr + $0.46 per mile.
- Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM
- (Weekend on-call rotation required)
- Productivity Benchmark: 30 points weekly.
HOME HEALTH RN CASE MANAGER | SUMMARY AND HIGHLIGHTS:
The Home Health RN Case Manager will act as a vital link between physicians, social workers, and families to provide high-quality, cost-efficient home health services. This role involves performing clinical assessments, developing individualized treatment plans, and managing transitions of care to ensure optimal patient outcomes.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH RN CASE MANAGER | RESPONSIBILITIES:
- Case Management: Perform comprehensive assessments of high-risk members and collaborate with primary care providers to implement individualized care plans.
- Clinical Intervention: Provide skilled nursing care, including wound care, IV administration, drain care, and G-tube management.
- Care Coordination: Manage patient transitions between care levels and coordinate with specialty resources, discharge planners, and social workers.
- Advocacy & Support: Identify non-medical barriers to care (such as housing or transportation) and implement supports to ensure treatment plan compliance.
- Outcome Tracking: Maintain detailed clinical and functional records to monitor fiscal and health outcomes during the management process.
- Preventative Care: Identify opportunities for health promotion and illness prevention to minimize adverse patient occurrences.
HOME HEALTH RN CASE MANAGER | REQUIREMENTS:
- Education: Associate’s or Bachelor’s Degree in Nursing.
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Licensure: Current, unrestricted RN license in Indiana (Compact license accepted).
- Ability to analyze complex medical information and make rational decisions.
- Case Management certification is highly preferred.
- Preferred Skills: Proficiency in wound care, IVs, drain care, G-tubes, and patient education.
- Software: Experience with Homecare Homebase (HCHB) is preferred.
- Ability to cover Harrison, Crawford, Orange, partial Floyd, and Washington counties (mileage reimbursement provided).
