Pay: $64/hour
Location: Bangor, ME 04401 (On-site/field-based home visits)
- Hours: 40 hours/week
- On-call: Required (may include evenings/weekends)
- Productivity expectation: 30 units/week
- Driving: Local travel required (reliable transportation + driver’s license + insurance)
Overview
We’re hiring an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their residences, coordinate care with an interdisciplinary team, and support patients/families through education and ongoing plan-of-care management.
Key Responsibilities
- Complete comprehensive patient assessments in stable and emergent situations
- Develop, implement, document, and evaluate the Plan of Care (POC)
- Maintain accurate, timely clinical documentation and patient records
- Provide patient/family education (medications, home safety, self-care, etc.)
- Coordinate with physicians, team members, and other healthcare professionals
- Supervise/guide home health aides as needed
- Follow infection control and safety standards (OSHA)
- Support compliance with third-party payer requirements (as applicable)
Job Requirements
Required Skills & Qualifications
- Active RN license (state license required). BSN preferred
- 1-2+ years RN experience in a clinical care setting. 2+ years Home Health experience
- OASIS experience & Homecare Homebase (HCHB) experience (required)
- CPR certification (current)
- Valid driver’s license + reliable transportation + insurance
Preferred
- CHPN certification
