Job Description
- Administer skilled nursing care for clients in their place of residence.
- Assess home health and hospice patients to identify their needs.
- Develop, document, and evaluate the Plan of Care (POC) for quality care.
- Educate patients and families on care, diet, medications, and safety.
- Supervise HHA/STNA/LVN to ensure quality service delivery.
- Monitor compliance with third-party payer requirements.
- Provide effective communication among all stakeholders, including patients and healthcare professionals.
- Shift: M-F until shift is complete; on-call required as necessary.
- Patient Service Area: Anderson County.
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- Current state license as a Registered Nurse.
- CHPN certification - Preferred.
- Proof of current CPR certification.
- 1-2 years experience as a Registered Nurse in a clinical care setting is required.
- 2+ years home health experience is required.
