Job Description
- Administer skilled nursing care for clients of all ages in their place of residence.
- Coordinate care with the interdisciplinary team, patient/family, and referring agency.
- Assess home health and/or hospice patients to identify their needs.
- Implement, develop, document, and evaluate the Plan of Care (POC).
- Educate patients and families regarding care, medications, and home safety.
- Supervise and provide clinical directions to home health aides.
- Monitor assigned cases for compliance with third-party payer requirements.
- Flexible schedule, travel locally required.
- No on-call responsibilities.
- Current state license as a Registered Nurse.
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- CHPN certification preferred.
- Proof of current CPR certification.
