Job Description
- Administer skilled nursing care for clients of all ages in their place of residence.
- Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure continuity of care.
- Educate patients and/or their families regarding care, diet, medications, and home safety.
- Supervise and provide clinical directions to home health aides to ensure quality service.
- Monitor assigned cases for compliance with third-party payer requirements.
- Provide effective communication and act as a liaison to patients, family, team members, and other health care professionals.
- On-call responsibilities required.
- Work hours may include evenings and weekends.
- Home health care environment.
- Current state license as a Registered Nurse.
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- 1-2 years of experience as a Registered Nurse in a clinical care setting.
- CHPN certification preferred.
- CPR certification required.
