Job Description
- Administer skilled nursing care for clients of all ages in their residence.
- Coordinate care with the interdisciplinary team, patient/family, and referring agency.
- Assess home health patients to identify physical, psychosocial, and environmental needs.
- Implement, develop, and evaluate the Plan of Care (POC) to ensure quality care.
- Educate patients and families regarding care, diet, medications, and home safety.
- Supervise and provide clinical directions to support staff to ensure continuity of service.
- Provide effective communication and serve as a liaison between patients, families, and healthcare professionals.
- On-call responsibilities; hours may vary.
- Work environment includes home settings.

- Current state license as a Registered Nurse is required.
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program is required.
- CHPN certification preferred.
