Job Description
- Administer skilled nursing care for clients of all ages in their place of residence.
- Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality care.
- Educate patients and/or families regarding care, diet, medications, and home safety.
- Supervise and provide clinical direction to HHA/STNA/LVN staff ensuring quality service.
- Monitor assigned cases to ensure compliance with third-party payer requirements.
- Maintain up-to-date patient records to reflect all interactions and care provided.
- No remote work; on-site care required in designated counties.
- Weekend on-call every other weekend/possibly every third weekend.
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- Current state license as a Registered Nurse.
- CHPN certification preferred.
- Proof of current CPR certification required.
- 1-2 years of experience as a Registered Nurse in a clinical care setting.
- 2+ years of home health experience required.
