Job Description
- Administer skilled nursing care for clients of all ages in their place of residence.
- Coordinate care with the interdisciplinary team, patient/family, and referring agency.
- Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC).
- Educate patients and/or their families regarding care, diet, and home safety.
- Supervise and provide clinical directions to the healthcare team to ensure quality service.
- Perform on-call responsibilities and provide on-call services as assigned.
- Schedule: Mon-Fri, no holidays, no weekends, no on-call.
- Coverage area includes Quad City area on both IA and IL (Rock Island IL, Moline IL, Davenport IA, Bettendorf IA).
- 36 hours guaranteed per week.
- Current state license as a Registered Nurse.
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- CHPN certification (preferred).
- Proof of current CPR certification.
- 1-2 years experience as a Registered Nurse in a clinical care setting.
- 2+ years of Home health experience.
