Job Description
- Administer skilled nursing care for clients of all ages in their place of residence.
- Coordinate care with the interdisciplinary team, patient/family, and referring agency.
- Assess home health and/or hospice patients in stable and emergent situations.
- Implement, develop, document, and evaluate the Plan of Care (POC).
- Obtain data on physical, psychological, social, spiritual, and cultural factors influencing health status.
- Ensure continuity of quality patient care with appropriate documentation.
- Provide care utilizing infection control measures.
- Educate patients and/or their families regarding care, diet, medications, and home safety.
- Supervise and provide clinical directions to HHA/STNA/LVN.
- Assess and reassess pain; utilize appropriate pain management techniques.
- Arrange referrals to appropriate agencies when needed.
- Monitor assigned cases for compliance with third-party payer requirements.
- Perform on-call responsibilities and provide services as assigned.
- Communicate effectively with patients, families, and healthcare professionals.
- Attend interdisciplinary team meetings as necessary.
- Perform other related duties as assigned or requested.
- Current state license as a Registered Nurse - Required
- 1-2 years experience as a Registered Nurse in a clinical care setting - Required
- 2 years Home health experience - Required
- Homecare Homebase Software Experience - Required
- OASIS Experience - Required
- Proof of current CPR certification - Required
- Current driver's license - Required
- Must have reliable transportation and insurance - Required
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
- Bachelors of Science in Nursing - Preferred
- CHPN certification - Preferred
