Case Manager / LTSS Assessor (RN) – Field-Based (Onondaga County to cover Onondaga and surrounding counties of Oneida, Madison and Cortland/ Senior Field RN Care Coordinator – UAS Assessments

Location: Syracuse, NY (Field-based)
Compensation: $50 – $58 per hour + mileage reimbursement 
Bonus: Up to 8% Annual Incentive
Employment Type: Full-time

We’re currently seeking a Senior LTSS Assessor (RN) for a field-based role focused on conducting UAS assessments in Onondaga County to cover Onondaga and surrounding counties of Oneida, Madison and Cortland. This position plays a key role in coordinating long-term care services for members with complex medical needs.

Position Purpose:

As a Senior LTSS Assessor, you will:

  • Perform comprehensive UAS assessments and reassessments in members' homes or other sites (2–4 daily)
  • Develop, implement, and monitor individualized long-term care plans
  • Coordinate medical and support services across the care continuum
  • Educate members, families, and caregivers on care options and resources
  • Collaborate with internal and external care teams to ensure quality, cost-effective, person-centered care
  • Precept and mentor new clinical hires, contributing to high-quality care and staff development

Qualifications:

Must-Haves:

  • Active Registered Nurse (RN) License in New York
  • Residency in Onondaga County to cover Onondaga and surrounding counties of Oneida, Madison and Cortland
  • Minimum 4 years of relevant RN experience
  • Experience conducting UAS assessments
  • Valid Driver’s License
  • Personal vehicle preferred, but public transportation will be considered
  • Comfortable being in the field Monday–Friday

Preferred Skills & Experience:

  • Bachelor’s degree in Nursing (BSN)
  • Experience in care management, ideally within a Managed Care Organization (MCO)
  • Familiarity with state and federal regulations
  • Strong understanding of community resources and care coordination
  • Ability to mentor and support clinical staff
  • Experience with high acuity/complex populations

Education & Experience:

  • Graduate from an Accredited School of Nursing required
  • Bachelor’s degree in Nursing (BSN) preferred
  • 4–6 years of nursing experience, ideally in care management or similar settings
