RN FIELD CASE MANAGER | DETAILS AND COMPENSATION:
- Location: Baltimore MD 21215 – 100% on-site, home health settings

- Will cover surrounding territoires
- Payrate: $56.14/hr + $0.45/mile
- Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM (On-call as needed)

- Caseload: Target of 30 points per week.
RN FIELD CASE MANAGER | SUMMARY AND HIGHLIGHTS:
The RN Field Case Manager will focus on performing comprehensive assessments for high-risk members and collaborating with primary care providers to implement individualized care plans. The ideal candidate is a strong communicator who is timely with documentation and passionate about patient advocacy.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN FIELD CASE MANAGER | RESPONSIBILITIES:
- Care Coordination: Collaborate with physicians, social workers, and discharge planners to transition patients to appropriate levels of care and engage specialty resources as needed.
- Assessment & Planning: Perform comprehensive clinical assessments and continuously reassess for cost-efficient, appropriate levels of care.
- Patient Advocacy: Act as a patient advocate while protecting privacy and confidentiality; implement non-medical supports (transportation, housing) to ensure plan compliance.
- Documentation: Maintain detailed records of clinical, functional, and fiscal outcomes; ensure all Oasis documentation is completed accurately and timely.
- Preventative Care: Identify opportunities for health promotion and intervene quickly to minimize adverse patient outcomes.
RN FIELD CASE MANAGER | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Licensure: Active, unrestricted RN License in the state of Maryland.
- Clinical Experience: Proven experience with Wound Care is required.
- Home Health Skills: Proficiency with Oasis documentation is required.
- Certifications: Valid CPR/BLS certification.
- Must possess a valid Driver’s License and active Auto Insurance for travel within Baltimore City, Baltimore County, and surrounding areas.
Preferred Qualifications
- Experience with Point of Care or HomeCare HomeBase (HCHB).
- Case Management Certification (CCM).
- Bachelor’s Degree in Nursing (BSN).
