RN FIELD CASE MANAGER - HOME HEALTH | DETAILS AND COMPENSATION:
- Location: Brunswick GA 31520 – 100% On-site

- Coverage area: Charlton, Camden, Glynn, and Brantley Counties
- Payrate: $56.14/hr + $0.48 a mile
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
RN FIELD CASE MANAGER - HOME HEALTH | SUMMARY AND HIGHLIGHTS:
The RN Field Case Manager will support high-risk members through comprehensive assessments, care coordination, and individualized care planning. This role focuses on ensuring appropriate, cost-effective levels of care while advocating for patient needs across clinical and non-medical support services.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN FIELD CASE MANAGER - HOME HEALTH | RESPONSIBILITIES:
- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Continuously assess and adjust appropriate levels of care
- Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals
- Arrange non-medical support services (e.g., housing, transportation) to promote treatment compliance
- Engage specialty resources as needed to optimize outcomes
- Maintain detailed clinical, functional, and fiscal documentation
- Identify opportunities for health promotion and illness prevention
- Prevent adverse patient events when possible and intervene promptly when risks arise
- Act as a patient advocate, ensuring privacy and confidentiality
RN FIELD CASE MANAGER - HOME HEALTH | REQUIREMENTS:
- Education: Associate’s or Bachelor’s degree in Nursing or related field
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Current, unrestricted Registered Nurse license (Georgia or Compact)
- Case Management certification required
- Strong ability to analyze complex medical information and make sound clinical decisions
- Preferred: Wound care, Catheter care, IV antibiotics administration, Lab draws, Drain management, Experience with HCHB software
