RN FIELD CASE MANAGER | DETAILS AND COMPENSATION:
- Location: Byram MS 39272 – 100% On-site
- Payrate: $56.14/hr + $0.49/mile reimbursement
- Required Availability: Full-Time | Monday–Friday, 8:00 AM–5:00 PM

- Rotating on-call coverage for weekends
RN FIELD CASE MANAGER | SUMMARY AND HIGHLIGHTS:
The RN Field Case Manager will support high-risk members through comprehensive care coordination and advocacy. This role focuses on assessing patient needs, developing individualized care plans, and collaborating with multidisciplinary teams to ensure cost-effective, high-quality outcomes.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance

RN FIELD CASE MANAGER | RESPONSIBILITIES:
- Perform comprehensive assessments for high-risk patients
- Develop and implement individualized care plans in collaboration with providers
- Coordinate care transitions and appropriate levels of care
- Collaborate with physicians, social workers, discharge planners, and other stakeholders
- Identify and address barriers to care, including non-medical support needs (e.g., transportation, housing)
- Maintain detailed documentation of clinical, functional, and fiscal outcomes
- Promote health education, illness prevention, and patient safety
- Act as a patient advocate, ensuring privacy and confidentiality at all times
RN FIELD CASE MANAGER | REQUIREMENTS:
- Education: Associate’s or Bachelor’s degree in Nursing or related field
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active, unrestricted RN license (compact license accepted)
- Strong clinical judgment and ability to analyze complex medical information
- Case Management certification (preferred)
- Preferred: Home health experience., OASIS documentation, Wound care, IV and drain management, HCHB (Homecare Homebase) experience
