RN CASE MANAGER - HOME HEALTH | DETAILS AND COMPENSATION:
- Location: Grand Junction CO 81506 – 100% On-site, Field Work

- Coverage: Mesa County
- Payrate: $56.14/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- One holiday per year on-call
- Average caseload: 14–16 patients; 20 visits per week expected
RN CASE MANAGER - HOME HEALTH | SUMMARY AND HIGHLIGHTS:
The RN Case Manager - Home Health will provide high-quality care coordination for high-risk patients. This role focuses on comprehensive assessments, individualized care planning, and collaboration with healthcare providers to ensure optimal patient outcomes.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance

RN CASE MANAGER - HOME HEALTH | RESPONSIBILITIES:
- Conduct comprehensive assessments of high-risk patients
- Collaborate with primary care providers to implement individualized care plans
- Continuously reassess and manage cost-efficient, appropriate levels of care
- Coordinate care transitions and engage specialty resources as needed
- Maintain detailed clinical, functional, and fiscal records throughout the care management process
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate, ensuring privacy and confidentiality
RN CASE MANAGER - HOME HEALTH | REQUIREMENTS:
- Education: Associate’s Degree in Nursing or related field
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Current, unrestricted RN license in Colorado (or multi-state license)
- Previous hospice or home health experience preferred
- Familiarity with electronic charting systems (HCHB/Pointcare) preferred
- Ability to analyze complex medical information and make sound clinical decisions
- Reliable transportation and valid car insurance
