RN CASE MANAGER | DETAILS AND COMPENSATION:
- Location: Jeffersonville IN 47130 – 100% On-site

- Counties Covered: Harrison, Crawford, Partial Floyd and Washington.
- Payrate: $56.14/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- On-Call: Weekend on-call rotation required
RN CASE MANAGER | SUMMARY AND HIGHLIGHTS:
The RN Case Manager will provide comprehensive care coordination for high-risk patients in a home health setting. This role focuses on clinical assessment, care planning, patient advocacy, and collaboration with interdisciplinary healthcare teams to ensure high-quality, cost-effective care.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN CASE MANAGER | RESPONSIBILITIES:
- Perform comprehensive assessment of high-risk patients
- Collaborate with primary care providers to develop and implement individualized care plans
- Identify, address, and continuously reassess appropriate levels of care
- Coordinate care transitions with physicians, social workers, and discharge planners
- Arrange non-medical support services (e.g., housing, transportation) to support treatment compliance
- Maintain detailed clinical, functional, and fiscal documentation
- Identify opportunities for health promotion and illness prevention
- Prevent adverse patient outcomes when possible and intervene promptly when needed
- Serve as a patient advocate while protecting privacy and confidentiality
RN CASE MANAGER | REQUIREMENTS:
- Education: Associates or Bachelor’s Degree in Nursing or a related field
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Current, unrestricted Registered Nurse license (Indiana or compact license accepted)
- Home health experience
- Case Management certification
- Ability to analyze complex clinical information and make sound decisions
- Valid driver’s license and reliable transportation
Preferred Skills
- Home health experience
- Wound care, IV therapy, drain care, and G-tube management
- Patient and caregiver education
- Experience with HCHB documentation systems
