RN FIELD CASE MANAGER | DETAILS AND COMPENSATION:
- Location: Lexington, KY – 100% On-site, field work/home health settings

- Coverage area to be assigned – 16 counties covered in total, candidate will not cover all counties
- Payrate: $49.21/hr + mileage
- Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM (includes some "on-call" requirements).
RN FIELD CASE MANAGER | SUMMARY AND HIGHLIGHTS:
The RN Field Case Manager will perform comprehensive assessments for high-risk members and collaborate closely with primary care providers to implement individualized care plans. This is a vital role focused on improving patient outcomes through advocacy, coordination, and clinical expertise.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN FIELD CASE MANAGER | RESPONSIBILITIES:
- Assessment & Planning: Conduct comprehensive assessments of high-risk members and develop individualized, comprehensive care plans.
- Coordination of Care: Collaborate with physicians, social workers, discharge planners, and claims professionals to ensure smooth transitions to appropriate levels of care.
- Advocacy & Support: Act as a patient advocate, ensuring privacy and confidentiality while coordinating non-medical support (e.g., housing, transportation) to improve treatment compliance.
- Outcome Management: Maintain detailed clinical, functional, and fiscal records; identify opportunities for health promotion and illness prevention.
- Resource Management: Engage specialty resources and reassess care levels to ensure cost-efficient, appropriate treatment.
RN FIELD CASE MANAGER | REQUIREMENTS:
- Education: Associate’s or Bachelor’s Degree in Nursing (or a related field).
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Must hold a current, unrestricted Kentucky Registered Nurse (RN) license.
- Ability to analyze complex medical information and make rational, clinical decisions.
Experience (Preferred):
- Previous experience with high-risk member assessments.
- Background in coordinating care transitions and collaborating with multi-disciplinary teams (Social Workers, PCPs, etc.).
