Job Description
  • Perform comprehensive assessment of high risk members.
  • Collaborate with primary care providers to implement individualized care plans.
  • Continuous reassessment of cost-efficient, appropriate levels of care.
  • Coordinate care transitions with physicians and other professionals.
  • Keep detailed records of clinical, functional, and fiscal outcomes.
  • Identify opportunities for health promotion and illness prevention.
  • Act as patient advocate by protecting privacy and confidentiality.
Critical Information
  • Shift Requirements: M-F 8am-5pm, On Call Required (1 weekend a month).
  • Remote/on-site/hybrid: Onsite.
Education/Licenses Needed
  • Current, unrestricted state Registered Nurse license (KY RN Licenses; Compact license accepted).
  • Associates or Bachelor’s Degree in Nursing or related field.
  • Case Management certification.
