RN FIELD CASE MANAGER - HOME HEALTH | DETAILS AND COMPENSATION:
- Location: Shiloh IL 62269 – 100% On-site

- Coverage area: St. Clair, Madison, Clinton, Randolph, Bond, Monroe, Portions of Washington Counties
- Payrate: $56.14/hr + $0.49 a mile
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- On-Call: Monthly rotation
RN FIELD CASE MANAGER - HOME HEALTH | SUMMARY AND HIGHLIGHTS:
The RN Field Case Manager will provide comprehensive care coordination for high-risk members. This role involves clinical assessment, care planning, and collaboration with interdisciplinary teams to ensure appropriate, cost-effective care while supporting positive patient outcomes.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN FIELD CASE MANAGER - HOME HEALTH | RESPONSIBILITIES:
- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to develop and implement individualized care plans
- Continuously assess and adjust appropriate, cost-efficient levels of care
- Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals
- Arrange non-medical support services (e.g., housing, transportation) to support treatment compliance
- Engage specialty resources as needed to achieve optimal outcomes
- Maintain detailed documentation of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Prevent adverse patient events when possible and intervene promptly when risks arise
- Serve as a patient advocate, ensuring privacy and confidentiality
RN FIELD CASE MANAGER - HOME HEALTH | REQUIREMENTS:
- Education: Associate’s or Bachelor’s degree in Nursing or related field
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Current, unrestricted Illinois Registered Nurse (RN) license
- Home health experience with strong clinical competency
- Experience with Homecare Homebase (HCHB) and OASIS documentation
- Ability to analyze complex medical information and make sound clinical decisions
- Case Management certification preferred
- Preferred: Lab draws, Admissions, recertifications, and resumptions of care, Ostomy care, Wound care and wound vacs, Tube feed management
