Home Health Registered Nurse (RN) Compensation
- The pay for this position is $56.14 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- The required availability for this position is Monday through Friday, 8:00 AM – 5:00 PM with participation in a monthly on-call schedule
- Mileage reimbursement is provided at $0.49 per mile
- Counties covered include St. Clair, Madison, Clinton, Randolph, Bond, Monroe, and part of Washington County, IL
- Perform comprehensive assessments of high-risk members in home settings
- Collaborate with primary care providers to implement individualized care plans
- Continuously reassess cost-efficient and appropriate levels of care
- Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
- Engage specialty resources as needed to optimize patient outcomes
- Arrange non-medical support services (housing, transportation) to ensure treatment compliance
- Maintain detailed clinical, functional, and fiscal documentation
- Identify opportunities for health promotion and illness prevention
- Prevent adverse patient occurrences and intervene promptly when necessary
- Act as a patient advocate, ensuring privacy and confidentiality
- Active, unrestricted Illinois RN license
- Associate’s or Bachelor’s Degree in Nursing or related field
- Case Management certification
- Ability to analyze complex medical information and make sound clinical decisions
- High School Diploma or GED
- Attendance is mandatory for the first 90 days
- Strong home health background with demonstrated competency
- Experience with infusion therapy, lab draws, admissions, recertifications, and resumptions of care
- Experience with ostomies, wound care, wound vacs, and tube feeds
- Experience with OASIS documentation
- Experience with Homecare Homebase
