RN CASE MANAGER | DETAILS AND COMPENSATION:
- Location: York PA 17402 – 100% On-site

- Counties Covered: Lancaster and York Counties
- Payrate: $56.14/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 4:30 PM, no weekends or on-call
RN CASE MANAGER | SUMMARY AND HIGHLIGHTS:
The RN Case Manager will provide comprehensive care management for high-risk patients. The role focuses on patient advocacy, clinical assessment, care coordination, and collaboration with interdisciplinary healthcare teams to ensure optimal outcomes.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN CASE MANAGER | RESPONSIBILITIES:
- Conduct comprehensive assessments of high-risk patients and members
- Develop and implement individualized care plans in collaboration with primary care providers
- Continuously reassess and address appropriate levels of care, including non-medical support such as housing and transportation
- Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
- Engage specialty resources to achieve optimal patient outcomes
- Maintain detailed clinical, functional, and fiscal documentation throughout the care process
- Identify opportunities for health promotion and illness prevention
- Prevent adverse patient outcomes when possible and intervene promptly when needed
- Serve as a patient advocate while maintaining privacy and confidentiality
RN CASE MANAGER | REQUIREMENTS:
- Education: Associates or Bachelor’s Degree in Nursing or a related field
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Current, unrestricted Registered Nurse license
- Case Management certification required
- Valid driver’s license and reliable transportation
- Ability to analyze complex medical information and make informed decisions
- Hospice and HCHB experience required
