RN CASE MANAGER | DETAILS AND COMPENSATION:
- Location: York, PA – 100% On-site, home health settings

- Counties Covered: Lancaster and York
- Payrate: $56.14/hr + mileage
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 4:30 PM

- Productivity: Expected to complete 5 to 7 visits per day.
- no weekend or on-call
RN CASE MANAGER | SUMMARY AND HIGHLIGHTS:
The RN Case Manager will collaborate with multidisciplinary teams to implement individualized care plans, ensuring all medical and non-medical support systems are in place to optimize patient comfort and dignity. This role requires high-level clinical decision-making and proficiency in HCHB to maintain detailed records of patient outcomes while meeting daily visit benchmarks.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN CASE MANAGER | RESPONSIBILITIES:
- Patient Assessment & Visits: Perform admission, recertification, routine, PRN, and death visits for hospice patients.
- Care Coordination: Collaborate with primary care providers, social workers, and discharge planners to implement individualized, comprehensive care plans.
- Resource Management: Identify and implement non-medical support (such as housing and transportation) to ensure compliance with treatment goals.
- Clinical Oversight: Continuously reassess cost-efficient, appropriate levels of care and identify opportunities for health promotion and illness prevention.
- Documentation: Maintain detailed records of clinical, functional, and fiscal outcomes using HCHB.
- Advocacy: Act as a patient advocate by protecting privacy and ensuring the highest standards of confidentiality.
RN CASE MANAGER | REQUIREMENTS:
- Education: Associate’s or Bachelor’s Degree in Nursing (ASN/BSN).
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Licensure: Current, unrestricted Pennsylvania Registered Nurse (RN) license.
- Certification: Valid BLS CPR certification required. Case Management certification is preferred.
- Experience: Previous Hospice experience is required. Must be comfortable performing admission and recertification visits.
- Travel: Must have a valid driver’s license and valid car insurance for travel throughout Lancaster and York counties.
- Skills: Ability to analyze complex medical information and make rational, patient-centered decisions.
