The Veterans' Service Unit is committed to providing high-quality care and support to veterans reintegrating into their communities. Our goal is to ensure that veterans receive the services they need to lead fulfilling lives, while adhering to state and federal guidelines. We are seeking a dedicated Re-Entry Social Worker to join our team and support our efforts in enhancing the lives of our veterans that are transitioning out of the correctional facility.
Position Overview:
The Re-Entry Social Worker will work specifically within the Veterans' Service Unit, providing vital support services to ensure veterans are receiving appropriate care and services. This role involves conducting monitoring reviews, evaluating service quality, and collaborating with stakeholders to address the needs of veterans in our care.
Minimum Qualifications:
- Bachelor of Social Work from an accredited social work program or equivalent education and experience.
- Basic computer skills, including proficiency in Microsoft Office products (Word, Excel, PowerPoint, Outlook).
- Strong communication and interpersonal skills.
- Ability to work collaboratively with diverse populations and stakeholders.
Key Responsibilities:
- Monitoring and Evaluation: Conduct on-site monitoring reviews of long-term living service providers to ensure consumers are receiving appropriate care that meets state and federal guidelines.
- Documentation Review: Review written documentation and engage with consumers and stakeholders to evaluate the quality of the services provided.
- Eligibility Monitoring: Monitor issues related to the medical and financial eligibility of consumers to ensure they have access to necessary services.
- Travel Requirements: Travel statewide as necessary to perform duties, including some overnight travel.
Pay and Hours:
- Pay: $28.00 per hour
- Hours: 8:00 AM - 4:00 PM
