Job Description
A-Line Staffing is now hiring a Senior LTSS Assessor (RN) in Albany, NY (Zip 12207) and Rensselaer, NY (Zip 12144)!
This is a full-time, field-based RN role working Monday through Friday, conducting 2-4 UAS Assessments daily. The position requires residency in Albany or Rensselaer County, NY. Mileage reimbursement and all necessary equipment will be provided.

- Pay Range: $35.49 – $63.79 per hour

Registered Nurse – Long-Term Services & Supports (LTSS) Position Requirements
Registered Nurse – Long-Term Services & Supports (LTSS) Must-Haves:
- NY Licensure as a Registered Nurse (RN)
- Residency in Albany (Zip 12207) or Rensselaer (Zip 12144) County
- Ability to conduct home or site visits (2–4 daily)
- Minimum 4 years relevant RN experience
- Driver’s license and personal transportation preferred
Registered Nurse – Long-Term Services & Supports (LTSS) Skills & Experience:
- Strong nursing skills; Bachelor’s degree in Nursing preferred
- UAS Assessment experience
- Ability to develop and manage care plans
- Strong communication and education skills with members, families, and caregivers
- Experience in care management, preferably within a managed care setting
- Knowledge of community resources and ability to coordinate services
Registered Nurse – Long-Term Services & Supports (LTSS) About the Role
As a Registered Nurse – Long-Term Services & Supports (LTSS), you will:
- Assess and coordinate medical and supportive services for high-acuity long-term care members
- Develop and monitor personalized care plans, collaborating with care management teams
- Serve as a liaison between members, caregivers, and healthcare providers
- Provide education and guidance to members and families
- Precept and coach new clinical hires
This position is onsite and field-based, servicing members in Albany (Zip 12207) and Rensselaer (Zip 12144) counties, Monday–Friday, 8:30am–5:00pm.
