Registered Nurse - Home Health

A-Line Staffing is currently seeking an experienced Registered Nurse for home health positions in the Loveland and Colorado Springs areas. Job Description: The primary function of the Registered Nurse (RN) is to administer skilled nursing care for patients of all ages in their home environment. The RN will coordinate care with the interdisciplinary team, the patient/family, and the referring agency, assuming responsibility for the coordination of care.

Job Name Schedule & Location:

• Loveland area (with occasional coverage in North Denver)
• Colorado Springs CO 80918 United States
• 13-week assignment
• On-call every 4-5 weekends
• 28-32 units per week
• Guaranteed 32 hours per week for Colorado Springs position

Job Name Salary/Wage:

• $40.00 to $52.00 per hour, based on experience

Job Name Duties:

• Assess home health and hospice patients in both stable and emergent situations.
• Develop and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Incorporate physical, psychological, social, and cultural factors into the POC.
• Ensure accurate documentation and maintain up-to-date patient records.
• Educate patients and families on care, medications, home safety, and self-care practices.
• Supervise and provide clinical direction to Home Health Aides (HHA), STNA, or LVN.
• Manage pain assessments and pain management techniques.
• Coordinate referrals to appropriate agencies.
• Monitor compliance with third-party payer requirements.
• Perform on-call responsibilities and provide services as assigned.
• Serve as a liaison between patients, family, team members, physicians, and other healthcare professionals.
• Participate in interdisciplinary team meetings.

Job Name Required Qualifications:

• 1-2 years of experience as a Registered Nurse in a clinical care setting.
• 2 years of home health experience required.
• Experience with Homecare Homebase software and OASIS required.
• Current state license as a Registered Nurse.
• Proof of current CPR certification.
• Must have a current driver’s license and reliable transportation with insurance.

Job Name Physical Requirements:

• Ability to function independently and manage multiple priorities under stress.
• Ability to work a flexible schedule and travel locally.

Job Name Education:

• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program (required).
• Bachelor of Science in Nursing (preferred).
