RN Care Manager Details:
- Location: Remote work with home visits in the San Antonio, TX area
- Schedule: Monday-Friday 8:00am-5:00pm
RN Care Manager Compensation
- The pay for this position is $38 - $43 per hour based on experience
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN Care Manager Responsibilities:
- Care Manager would be responsible to complete 6 assessments per week with our members in their home. Our position is with the Assessment Team and we are the ones to complete initial assessments for new members and we also complete reassessments for Level 3 members (low acuity). Our team also completes Medically Dependent Children’s Program Interest List assessments.
- Complete assessments, create and revise care plans, documentation in EHR (TruCare).
- Contact providers, members, family members as needed to complete aforementioned assessments, care plans and relevant documentation
RN Care Manager Qualifications:
- Active RN license in the state of Texas
- Care / Case Management experience
- Critical thinking and time management skills
- Computer skills are needed
- Some experience with Pediatrics preferred
- Experience in home health preferred
RN Care Manager Preferred Qualifications:
· Behavioral health background preferred
· Proficient with Microsoft programs, Outlook, Excel
· Medicaid and Medicare knowledge
