Location: Louisville, KY (Field-Based/Onsite)
Schedule: Monday–Friday, 8:00 AM–5:00 PM (Rotating weekend on-call)
Pay: $56.14/hr

Responsibilities
- Complete in-home skilled nursing assessments and ongoing patient evaluations
- Develop, implement, and update individualized Plans of Care (POC)
- Document accurately in HCHB and complete detailed OASIS assessments
- Provide skilled nursing care including wound care, IV care, drain care, G-tube care, and patient education
- Coordinate care with physicians, interdisciplinary team members, patients, and caregivers
- Support care transitions, referrals, and service coordination to the appropriate level of care
- Maintain compliance with payer requirements, quality standards, and HIPAA
- Participate in rotating weekend on-call coverage as assigned
Qualifications
- Active, unrestricted RN license (compact license accepted)
- Associate or Bachelor’s degree in Nursing (or related)
- Home health/field-based nursing experience strongly preferred
- Homecare Homebase (HCHB) experience required
- Strong OASIS documentation experience required
- Valid driver’s license, reliable transportation, and ability to travel locally
- Strong time management, independent workflow, and patient education skills
